Lumps in the neck, in front of the ear or at the jaw angle need careful assessment to know whether they are benign or malignant and whether surgery is needed. This is head and neck surgery, where important nerves must be protected.

When to see a doctor

Signs to watch for

  • A lump at the front of the neck, trouble swallowing, or hoarseness with no clear cause.
  • A thyroid nodule or goiter found on ultrasound.
  • A lump in front of or below the ear or at the jaw angle, growing slowly or quickly.
  • Weakness or numbness on one side of the face together with a lump near the parotid gland.
  • A lymph node or lump in the neck lasting more than a few weeks.
Process

Examination and treatment process

  1. 1Clinical examination of the neck and face, checking facial movement, voice and the lymph node groups.
  2. 2Ultrasound, blood tests and fine-needle aspiration when needed; CT or MRI for large or suspicious lumps.
  3. 3Benign nodules or lumps without symptoms may only need regular monitoring; when surgery is indicated the doctor explains the method, benefits and risks.
  4. 4Surgery is planned at a suitable medical facility with care to protect the recurrent laryngeal nerve and parathyroid glands (thyroid surgery) and the facial nerve (parotid surgery).

Good to know before the visit

  • Bring all ultrasound, blood test, pathology and imaging results.
  • Tell the doctor about radiation to the head and neck, thyroid disease in the family and current medicines.
  • Do not use supplements advertised to “dissolve nodules” before a diagnosis.

Frequently asked questions

Does a thyroid nodule mean cancer?

No. Most thyroid nodules are benign, but they need an ultrasound and assessment to decide which can simply be watched and which need a needle biopsy or surgery.

Are parotid tumors dangerous?

Most are benign, but some can be malignant or recur if not removed properly. A proper diagnosis is needed before deciding on treatment.

Can the surgery affect my voice or face?

The operations lie close to the voice nerve (thyroid) and the facial nerve (parotid). The doctor always tries to protect them and explains each person’s specific risks before surgery.
